ARTISTS REGISTRATION FORM

Please read this form carefully and if you have any questions raise them immediately with PES Ltd.
With immediate effect, PES Ltd undertakes to act as an intermediary processing payments between the self-employed artist agreeing to this document (service provider) and the individual who requires the service (service receiver).
By signing this document the service provider agrees that they will not hold PES Ltd liable for amounts that have not been paid to PES Ltd by the service receiver.

Declarations & Understanding
1. I am self-employed contractor. I am not an employee of PES Ltd or any of PES Ltd.'s clients.
2. I am solely responsible for my own liability to HM Revenue and Customs and any payments I receive from PES Ltd on behalf their clients will be gross of any applicable tax and/or NI contributions that I may be liable for.
3. I will provide a professional service at all times.
4. The artist assigned agree that PES ltd acts as an intermediary and cannot be held responsible for non-fulfillment of booking by the client although every safeguard is assured.
